# Medicare Covers 80%. Here's How to Cover the Rest.

**Here is one of the most useful things to know about Medicare, and one of the least explained:** Medicare generally covers 80% of your outpatient care. The other 20% is up to you. The good news: covering that 20% is simple, often very affordable, and exactly what secondary insurance was built for.

Picture the alternative for just a moment. A procedure, a recovery, and then the bills arrive: the remaining 20% of a $50,000 surgery, $10,000, lands on you, and with Original Medicare alone there is no yearly limit on that 20%. Planning is never just about today. It is about the future, the procedure you can't predict, the year you hope never comes. With secondary insurance in place, you plan better and find peace in knowing you are covered.

Here is the whole picture, in plain language, plain numbers, and no jargon.

## First, Let's Decode the Letters

Medicare uses letters in two different ways, and this is where most of the confusion starts. Here is the simple version: **Parts A, B, C, and D are the pieces of Medicare itself.** Which pieces you have decides whether secondary insurance fits.

| Letter | What it is | Can you add secondary insurance? |
| --- | --- | --- |
| **Part A** | Hospital coverage. One half of Original Medicare. | **Yes.** A and B together are what secondary insurance pairs with. |
| **Part B** | Doctor visits and outpatient care. The other half of Original Medicare, and the home of the 80/20 split below. | **Yes.** Same pairing: Original Medicare plus a supplement. |
| **Part C** | Medicare Advantage: the all-in-one private alternative to Original Medicare. | **No.** Supplements can't be combined with Advantage plans. Instead, these plans use copays and include a yearly out-of-pocket maximum. |
| **Part D** | Prescription drug coverage. | **It runs alongside.** People with Original Medicare plus a supplement usually add Part D for medications. |

**One more alphabet, and then you're fluent:** secondary insurance plans, called Medicare Supplements or Medigap, use letters too, like **Plan G** and **Plan N**. Those letters are simply the names of standardized packages, the way cars have trim levels. Every company's Plan G is identical by law, so the letter tells you the coverage and the only difference is the price.

## The 80/20 Picture

With Original Medicare, Part B generally pays **80%** of the Medicare-approved amount for doctor visits and outpatient care after your deductible. The other **20%** is yours, and that is exactly the gap secondary insurance exists to fill.

With secondary insurance (such as a Medicare Supplement, retiree coverage, or Medicaid):

With Original Medicare alone:

| 80% | 20% |

■ Medicare pays   ■ You pay, out of pocket, with no yearly cap

**Why planning ahead matters:** a single serious diagnosis, surgery, or hospital stay can turn that uncapped 20% into tens of thousands of dollars. Secondary coverage, whether a Medicare Supplement (Medigap) plan, coverage from a former employer, or Medicaid for those who qualify, picks up some or all of that 20%, and with the most comprehensive plans, your care is covered up to 100% of Medicare-approved costs.

## When Can You Get Secondary Insurance?

Here is the part almost nobody explains. Secondary insurance has its own timing rules, separate from the fall Medicare window, and they reward acting early.

Your golden window: the first 6 months

When you are 65 or older and your Part B coverage begins, you get a one-time, six-month window where companies **must** accept you, at the standard price, with no health questions. This is the easiest and best-protected moment to sign up, and it happens once.

After the window: any time of year

You can still apply whenever you like, there is no annual season. The difference: in most states, companies can now ask health questions, charge more based on your history, or decline the application. This is why sooner beats later.

The helpful exceptions

Certain events open protected windows at any age and any time, such as losing employer coverage, your Medicare Advantage plan leaving your area, or your first year after trying an Advantage plan. And some states add friendlier rules of their own: California's birthday rule, for example, lets you switch to equal or lesser coverage around your birthday each year with no health questions. A licensed professional or SHIP counselor can tell you exactly which rights apply in your state.

**The simple takeaway: earlier is easier. The best time to get covered is while you are healthy, and the second-best time is today.**

## What Does Secondary Insurance Cost?

Often less than people expect, and far less than one uncovered procedure. Here are typical 2026 monthly premium ranges for the most popular Medicare Supplement plans at age 65. Your quote depends on your age, state, and the company you choose.

| Plan | Typical monthly cost | Good to know |
| --- | --- | --- |
| **Plan G** | **$110 to $220** | The most comprehensive option for new enrollees; covers nearly everything except the small Part B deductible |
| **Plan N** | **$85 to $170** | Lower premium in exchange for small copays at the doctor and emergency room |
| **High-Deductible Plan G** | **$40 to $70** | Very low premium; you pay costs up to a $2,950 deductible (2026) before it pays, then you are protected |

Companies that sell Medicare Supplement plans in most states include AARP/UnitedHealthcare, Mutual of Omaha, Aetna, Cigna, Humana, and your state's Blue Cross Blue Shield plan, among others. And here is the part that saves real money: **plans are standardized by letter, so every Plan G has identical benefits by law.** The same coverage can cost $50 to $100 a month more at one company than another, which means the smartest move is comparing prices from at least three companies before you choose.

**Where to get pricing:** a licensed independent agent can quote multiple companies at once at no cost to you, your SHIP counselor (below) can review quotes with you without selling anything, and Medicare.gov has a [Find a Medigap Policy](https://www.medicare.gov/medigap-supplemental-insurance-plans/) tool that shows plans and price ranges in your zip code.

October 15 to December 7, 2026 · Changes take effect January 1, 2027

The fall window is when everyone with Medicare can review their Medicare Advantage and Part D choices for the year ahead, and it is the season when most families finally sit down and look at the whole coverage picture, secondary insurance included. If someone is moving from an Advantage plan back to Original Medicare this fall, that is exactly the moment the supplement rules above come into play. Use the simple plan below and walk in prepared.

October 15

Open Enrollment begins. You can join, switch, or drop a Medicare Advantage or Part D prescription drug plan, or move between Original Medicare and Medicare Advantage.

December 7

The last day to make your changes. After this, most coverage is locked in for the year.

↓

January 1, 2027

Your new coverage takes effect for the year ahead.

## Your 7-Day Prep Plan

Pick a start day this week and take one step a day. Each one takes minutes, and together they turn an overwhelming season into a confident decision.

**Day 1 · Gather the basics.** Your Medicare card, a current list of your medications with doses, and the names of the doctors you want to keep.

**Day 2 · Read your Annual Notice of Change.** Your current plan mailed it by September 30. It spells out exactly what changes in 2027: premiums, drug coverage, doctor networks.

**Day 3 · Note what changed in your life.** New diagnoses, new prescriptions, a new doctor, more travel, a move. Your plan should fit the year ahead, the one behind you.

**Day 4 · Preview 2027 plans.** Visit the official Plan Finder at [Medicare.gov](https://www.medicare.gov/plan-compare) and see what is available in your zip code.

**Day 5 · Compare the real costs.** Look past the monthly premium to deductibles, copays, drug tiers, and whether your pharmacy and doctors are in network, and ask about the 20%.

**Day 6 · Book your free help.** Reserve time with a Medicare counselor or licensed professional (all the ways to reach one are below). Appointments fill fast once the window opens.

**Day 7 · Write your questions.** Three to five questions, on paper. On October 15 you will act with a plan while everyone else is still scrambling.

## Medicare and State Programs: What's the Difference?

These two get mixed up constantly, and knowing the difference can save you money and stress, wherever you live. Here it is in plain language.

| Medicare | Medicaid (your state's program) |
| --- | --- |
| **Federal** health insurance, the same core program in all 50 states | **Run by each state** with federal support, so rules and names vary. California calls it Medi-Cal, other states have their own names |
| Based on **age or disability**: 65 and over, or younger with certain conditions | Based on **income and need**, at any age |
| Covers hospital care, medical care, and prescription drugs through its parts and plans | Can cover what Medicare largely does not, including **long-term care** and many out-of-pocket costs |
| Enrolls during **set windows**, like the one opening October 15 | Enrolls **year-round**, whenever you qualify |

The helpful in-betweens

Many people qualify for both, called being **dual eligible**. And even if your income is too high for full Medicaid, **Medicare Savings Programs** and the **Extra Help** program can pay Medicare premiums and prescription costs for those who qualify. These are applied for through your state, any time of year, and millions of eligible people never claim them. It costs nothing to ask.
